2018 Noches de Havana Silent Auction Donation Form

Submit donations for the Silent Auction.
  • All donor forms, items and display/advertising materials must be received by Friday, March 23, 2018.

  • Please provide information as it should appear in the event materials:
  • Phone and Extension, if any.
  • Please give a detailed description of the donated item(s) and their contents. Include all applicable restrictions. Gift certificates should be valid from Saturday, April 21, 2018 to at least six months after the event date.
  • (Required for IRS purposes and for donation receipt)
  • Let us know when the donation will be ready to pickup or when to expect delivery if sending.
    Display materials include catalogs, menu, business cards, poster, shopping bag, or any other branded or advertising materials that can be placed alongside your donation.
